Building a Bridge to Better Outcomes

The transition from hospital to community settings is a critical time. Challenges for stroke survivors and care partners do not end when they leave the hospital. They require customized care and support.

Kandu Health provides stroke survivors with the guidance they need at home with a virtual recovery support.  We partner with hospitals to ensure patients have a safe transition home and a customized recovery plan. Together, we provide continuity of patient care to achieve better quality of life for stroke survivors and their care partners.

Navigating the Transition: Challenges in Stroke Recovery Post-Inpatient Care

Stroke survivors face significant barriers after discharge, including physical impairments, chronic conditions, mental health challenges, and fragmented care systems. This session will explore the high prevalence of neuropsychological symptoms, treatment gaps, and their impact on recovery, readmission rates, and quality of life. Using recent data, this session will highlight the critical need for interprofessional collaboration to address these issues and improve outcomes.

PINpointing Recovery: Personalized Approaches to Complex Post-Acute Stroke Navigation

This session will explore the transformative potential of Principal Illness Navigation (PIN), a CMS-supported care coordination model, addressing the complex needs of stroke survivors through individualized, navigator-driven support. This session will showcase the benefits of PIN for patients, hospital teams, and payers, including improved outcomes and reduced healthcare utilization. Insights from a telemedicine-based post-stroke recovery program will highlight its survivor-focused impact.

Empowering Stroke Survivors Through Digital Health Based Navigation

This poster examines the experiences of stroke survivors that received personalized education, clinical navigation, and community connections to address critical health and behavioral needs. The results underscore the program's potential to improve recovery outcomes, promote person-centered care, and reduce healthcare disparities across diverse populations.

Kandu Stroke Recovery Services Provide Support for Stroke Survivor Readmission Prevention

Kandu's services provide remote post-discharge stroke recovery support through clinically-licensed Navigators that increases stroke survivors' quality of life, creates durability of clinical outcomes created by the acute provider, and decreases subsequent ED visits and readmissions. To date, Kandu has helped reduce readmission rates of enrollees by over 50% compared to published rates (data on file at Kandu Health).
